A fully completed and SIGNED claim notification form;
A CLEARLY legible certified copy of the ID document/Smart ID Card of the deceased; Must have a CANCELLED STAMP ON SHOWING DECEASED FROM THE DEP OF HOME AFFAIRS
A CLEARLY legible certified copy of Death Certificate;
A CLEARLY legible certified copy of the ID document of the Claimant;
If no appointed beneficiary, an affidavit nominating a new beneficiary together with a certified copy of ID of new Beneficiary
In the case of claiming for a child: A certified copy of the child’s birth certificate, guardianship letter, if applicable
Proof of Bank Account Number of claimant/ ONLY 1ST PAGE OF BANK STATEMENT
Proof of marriage when applicable. (When main member claims for spouse or spouse claims for main member)

Same documents applicable as with Natural Causes
A police report on all UNNATURAL deaths
PLEASE NOTE THAT NO CLAIM WILL BE ASSESSED UNTIL ALL RELEVANT, CORRECTLY FILLED IN DOCUMENTS, HAVE BEEN RECEIVED BY THE ADMINISTRATOR. ALL COPIES MUST BE CLEAR AND ALL COPIES MUST BE CERTIFIED BY A COMMISSIONER OF OATH.
